 Staff Pharmacist - Full-Time

Under the general supervision of the Director of Pharmacy, the Staff Pharmacist, accurately reviews and interprets medication orders; prepares accurate and complete patient profiles; reviews profiles; reviews profiles for correct drug, dosage of administration; ensures that no drug duplication or drug-drug interaction exists; ensures that patient is not allergic to any medication prescribed.  Efficiently performs other professional pharmacy duties including the safe procurement, preparation, labeling, distribution, and control of all drugs.   Notifies appropriate medical/nursing staff whenever appropriate drug therapy is questionable.  Actively supports and participates in pharmacy activities recommended by The Joint Commission or other regulatory agencies. 

 The incumbent will carry-out duties by adhering to the highest standards of ethical and moral conduct, acts in the best interest of Three Crosses Regional Hospital, and fully supports the mission, vision, and values of Three Crosses Regional Hospital.

Responsibilities

  • Interprets, clarifies and transcribes all medication orders accurately and timely.   Documents medication order clarification.  
  • Monitors medication orders and reviews patient profile to ensure appropriate drug selection and dosage (no drug duplication, interactions or allergies).   Documents recommendations.  
  • Prepares and labels all drugs accurately and timely.
  • Actively participates in the Adverse Drug Reaction Reporting Program. 
  • Demonstrates working knowledge of pharmacy’s policies and procedures. 
  • Supports the formulary development process by consulting with medical staff and promoting rational drug therapy selection.
  • Supports pharmacy management in inventory control and purchasing procedures. 
  • Issues controlled substances to patients and maintains records as required by appropriate authorities.
  • Demonstrates ability to function as a team member in performance improvement and medication use review activities. 
  • Maintains competency required for current Job Description and Demonstrates knowledge in all areas identified in the Competencies.  A copy of this completed/graded competencies will accompany the Evaluation.
  • Provides drug information/education to patients and their families as required by legislative/regulatory authorities.
  • Demonstrates ability to set priorities, work independently and cooperatively with others within and outside the department.
  • Perform other duties as assigned by appropriate management.
  • Attend staff meetings as needed.
  • Other duties as assigned.

Education and Experience:

  • Required: Bachelor’s Degree in Pharmacy or Pharm.D
  • Preferred: 2-4 years of experience as staff pharmacy health care environment.
  • Required: Meet all Employee Health Requirement
  • Required: Current License as required by State Board of Pharmacy.
  • Preferred: Current BLS and CPR Certification
