Position Overview:

The Health Programs Manager is responsible for assisting the Health Programs Director in achieving the goals of the Health Program Department in their applicable region.

The Health Programs Manager supervises the Health Educators and Community Health Workers working on various projects. Ensures the effective and excellent performance of educators and promotoras by supporting, training, coaching, directing, and evaluating the work of team members individually and as a group, maintaining staff schedules, and overseeing and approving time and attendance records.

What you will do:

 

  • Demonstrates excellent oral, written, and interpersonal communication skills alongside intermediate proficiency with PC hardware and MS Office (Word and Excel).

  • Collaborates effectively in a team environment and participates in cross-training opportunities under the ECFH Code of Ethics.

  • Follows PCMH, Meaningful Use, and evidence-based clinical guidelines using certified EHR systems.

  • Manages, trains, coaches, and evaluates Health Educators and Community Health Workers (promotoras), including overseeing staff schedules and timecard approvals.

  • Delivers high-quality customer service that respects patient rights, needs, and confidentiality, while providing technical assistance to providers on project procedures.

  • Delivers direct individual and group health education, health promotion, and disease management activities, while managing all patient education materials.

  • Implements integrated care, referral, and tracking systems across ECFH programs, utilizing data analysis and statistical criteria to track patient outcomes.

  • Facilitates fitness calls for staff wellness programs and referred patients.

  • Pursues continuing education to update technical skills, attends external meetings with collaborating agencies, and maintains communication across all department projects.

 

Minimum requirements

Education: Bachelor's degree in health education or closely related field from an accredited institution.

Experience: Minimum of three years experience as a health educator in clinical, community or public health setting. Supervisory and program management experience.

Certifications: Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR).
