Position Summary
The Referral Coordinator is responsible for the coordination and processing of patient referrals for specialty services, diagnostic testing, and other outpatient services for multiple primary care providers, obtaining insurance authorization when needed, and maintaining ongoing tracking and documentation of referral coordination activities and status.

Essential Duties and Responsibilities
- Provides exceptional customer service to both internal and external customers.
- Coordinate all primary care referrals to designated site (e.g., specialty services, diagnostic testing, and other outpatient services)
- Act as a patient liaison/advocate, to ensure optimal patient experience in assessing NVHC services and referred care
- Obtain prior authorization as needed
- Attend required meetings and participate in committees as requested.
- Completes appropriate patient documentation in electronic medical records.
- Maintain constant and proactive communication with the patient regarding referral
- Assist patients in problem solving potential issues with insurance, financial, or social issues
- Accurately maintain referral network contact information
- Alerts for ways to improve customer service, improve patient flow
Required Qualifications
- High school diploma or general education degree (GED)
- 1 year medical office experience
- Strong communication skills including written and verbal communication
Preferred Qualifications
- Associate degree preferred
- English/Spanish fluency
- 1 year experience with insurance carriers, prior authorizations, and/or coordination of care
- Demonstrated proficiency with computer programs and data entry, experience with Electronic Health Records highly preferred
- Previous experience using a multi-line phone system
- Ability to manage time effectively
