Job Summary

Delivers individualized PT services including assessment, treatment planning, implementation, documentation, and education to achieve functional goals.

Minimum Qualifications

• Bachelor’s (or higher) in PT from CAPTE‑accredited program; current state license; 1 year recent clinical experience.

Preferred Qualifications

• CPR certification may be required.

Essential Duties & Responsibilities

• Evaluate function; develop/revise plans of care.

• Direct PT support staff; educate patients/families.

• Document assessments/goals/progress/discharge.

• Collaborate with interdisciplinary team; ensure compliance.

Physical Requirements

• Demonstrate and assist with therapeutic exercises and functional mobility.

• Frequent standing, walking, bending, reaching, and kneeling.

• Lift, support, and transfer patients with assistance/devices as needed (up to 50 lbs independently).

• Manual dexterity for therapeutic techniques and equipment; adequate vision and hearing.
