JOB SUMMARY

An Occupational Therapist (OT) administers occupational therapy to patients in their place of residence. This is performed in accordance with physician orders and IDG plan of care under the direction and supervision of the Manager of Patient Services/Director.

II. DUTIES & RESPONSIBILITIES

路 Minimizes residual physical disabilities of the patient.

路 Periodically participates with the IDG and all other health care personnel in patient IDG care planning.

路 Provides prescribed occupational therapy.

路 Directs and supervises personnel as required.

路 Performs all procedures as ordered by physician and IDG according to the therapy plan of care.

路 Consults with physicians and IDG regarding change in treatment.

路 Instructs patients and family members in home programs and fine motor movement exercises.

路 Periodically presents an in-service to Hospice staff.

路 Assists the physician in and IDG evaluating level of function.

路 Prepares clinical and progress notes.

路 Advises and consults with the family and other Hospice personnel.

路 Participates in in-service programs.

III. JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)

路 Is a licensed or otherwise regulated, if applicable, as an OT by the State in which practicing, unless licensure does not apply途

路 Graduated after successful completion of an OT education program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) of the American Occupational Therapy Association, Inc. (AOTA), or successor organization of ACOTE途 and

路 Is eligible to take, or has successfully completed the entry level certification examination for OT developed and administered by the National Board for Certification in Occupational Therapy, Inc. (NBCOT).

On or before December 31, 2009:

路 Is licensed or otherwise regulated, if applicable, as an OT by the State in which practicing途 or

When licensure or other regulation does not apply:

路 Graduated after successful completion of an OT education program accredited by the Council for Occupational Therapy Education (ACOTE) of the American Occupational Therapy Association, Inc. (AOTA) or successor organization of ACOTE途 and

路 Is eligible to take, or has successfully completed the entry level certification examination for OT developed and administered by the National Board for Certification in Occupational Therapy, Inc., (NBCOT).

On or before January 1, 2008:

路 Graduated after successful completion of an OT program accredited jointly by the committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association途 or

路 Is eligible for the National Registration Examination of the American Occupational Therapy Association or the National Board for Certification in Occupational Therapy.

On or before December 31, 1977:

路 Had 2 years of appropriate experience as an OT途 and

路 Had achieved a satisfactory grade on an OT proficiency examination conducted, approved or sponsored by the U.S. Public Health Service.

If educated outside the U.S., must meet both of the following:

路 Graduated after successful completion of an OT education program accredited as substantially equivalent to OTA entry level education in the U.S. by one of the following:

- The Accreditation Council for Occupational Therapy Education (ACOTE).

- Successor organizations of ACOTE.

- The World Federation of Occupational Therapists.

- A credentialing body approved by the American Occupational Therapy Association.

路 Successfully complete the entry level certification examination for OT developed and administered by the National Board for Certification in Occupational Therapy, Inc. (NBCOT).

路 On or before December 31, 2009, is licensed or otherwise regulated, if applicable, as OT by the State in which practicing.

路 The ability to drive and/or have access to transportation for agency related travel (e.g. community partners, patients, etc.) is an essential function of the job. Must have and maintain a valid driver鈥檚 license, maintain automobile insurance coverage and have access to an automobile.

路 Two (2) years of experience, preferred.
