What You'll Do

  • Evaluate patients in their homes and develop individualized occupational therapy treatment plans.
  • Improve independence with activities of daily living, home safety, upper extremity function, cognition, and adaptive techniques.
  • Recommend home modifications and adaptive equipment that support long-term independence.
  • Educate patients and caregivers on strategies that improve safety and function.
  • Collaborate closely with physicians and the interdisciplinary care team to achieve outstanding patient outcomes.
  • Complete timely, accurate clinical documentation.

What We're Looking For

  • Active Minnesota Occupational Therapist (OTR/L) license.
  • Strong clinical reasoning and patient-centered care philosophy.
  • Excellent communication and organizational skills.
  • Home health experience is helpful but not required.
  • Valid driver's license and reliable transportation.
