Overview

Health at Home Nurse Practitioner (FT) APRN

 

The Nurse Practitioner (APRN) is a registered professional nurse with advanced graduate education and clinical training who supports comprehensive home-based care services with a focus on end-of-life care, including palliative care and hospice, in compliance with the state nurse practice act. The APRN conducts holistic assessments of assigned patients to contribute to the overall evaluation of the patient’s care needs and the patient’s goals of care. Participates as part of the hospice medical team by completing face-to-face visits, acting as an attending provider and is a contributing member of the hospice interdisciplinary team. Serves as an active member of the Section of Palliative Medicine at Dartmouth-Hitchcock, with an academic appointment in the Department of Medicine, participating in the education of Hospice and Palliative Medicine fellows and rotating learners, and collaborating with Palliative Medicine team members in the care of mutual patients.

Qualifications

  • Board-certification by appropriate accrediting body to practice as an APRN.
  • Must have an active national certification as an Adult Nurse Practitioner, Adult- Gerontology Primary Care Nurse Practitioner or Family Nurse Practitioner.
  • The ideal candidate will have completed advanced practice fellowship training in palliative care, OR will have at least 2+ years of experience practicing in hospice palliative care.
  • Certification in Advanced Practice Palliative Nursing or be eligible for certification within one year.
  • Eligible for NH and VT licenses and DEA  

Responsibilities

  • Evaluate, assess and manage Palliative Care needs, including: pain and symptom management, coping with serious illness in patients and family. Elicit understanding of illness and information preferences, share prognostic information, and elicit values and goals. Where appropriate, refer to other members of the interdisciplinary team, such as nursing, social work, or chaplaincy.
  • Provide education to the patients and their family, according to the plan of care, to facilitate optimal patient and family support relative to the patient's illness.
  • Work effectively with other health care professionals in environments where we may see patients, including patient homes, skilled nursing facilities etc.
  • Assist with diagnosis, treatment and management of acute and chronic health conditions.
  • Consult with Chief Medical Officer as needed, inform primary provider of recommended services and collaborate with other providers as needed.
  • Educate facility staff, co-workers and the community on all aspects of home-based care programs, including palliative care and hospice, as well as services provided through Dartmouth Health at Home.
