Job DetailsJob Location: Main Street Building - Leesburg, FL 34748Position Type: Full TimeEducation Level: 4 Year DegreeSalary Range: $19.78Job Purpose: - The mobile response team helps to reduce the impact of mental health emergencies through immediate response to crisis situations at the street level and through coordination and consultation with local public safety organizations, hospitals, and other community groups. The Case Manager II a is responsible for providing comprehensive case management services to individuals with behavioral health needs. The role involves coordinating care, advocating for clients, and ensuring they receive appropriate services to improve their quality of life and achieve their personal goals. Key Responsibilities: - Conduct comprehensive assessments of clients to identify their needs, strengths, and goals. - Develop, implement, and monitor individualized care plans in collaboration with clients and other stakeholders. - Coordinate with healthcare providers, social services, and community resources to ensure clients receive necessary services and support. - Advocate for clients to access appropriate resources and services, addressing any barriers to care. - Maintain accurate and up-to-date client records, documenting all interactions and progress. - Provide crisis intervention and support as needed, ensuring client safety and well-being. - Facilitate group sessions and workshops to promote client education and skill development. - Participate in multidisciplinary team meetings to discuss client progress and adjust care plans as necessary. - Stay informed about changes in behavioral health services, policies, and regulations to ensure compliance and best practices. - Engage in continuous professional development to enhance skills and knowledge in case management and behavioral health. QualificationsRequired Education: - Bachelor’s degree in Social Work, Psychology, Counseling, or a related field from an accredited institution. Required Experience: - Minimum of 2 years of experience in case management or a related field within a behavioral health setting. - Experience working with diverse populations, including individuals with mental health challenges, substance abuse issues, or developmental disabilities. - Demonstrated experience in developing and implementing individualized care plans. - Proven track record of coordinating with multidisciplinary teams to deliver comprehensive care services. Required Skills and Abilities: - Strong understanding of behavioral health principles and practices. - Excellent communication skills, both verbal and written, with the ability to interact effectively with clients, families, and professionals. - Ability to assess client needs, develop treatment plans, and monitor progress. - Proficient in using case management software and maintaining accurate documentation. - Strong organizational skills with the ability to manage multiple cases simultaneously. - Ability to work independently as well as collaboratively within a team environment. - Demonstrated problem-solving skills and the ability to make sound decisions under pressure. - Knowledge of local community resources and services available to clients.
