Community Living & Care Coordination
Location: Leominster, MA
Schedule: Part-Time (21 hours per week)
Hours: Monday, Tuesday, and Wednesday | 9:00 AM – 5:00 PM
Compensation: $22-$24 per hour

Aging Services of North Central Massachusetts (ASNCM) is seeking a Community Transition Specialist to join our mission-driven team.
In this rewarding role, you will help older adults transition from nursing facilities back into the community. Working closely with consumers, families, healthcare providers, and housing agencies, you will play an important role in helping people achieve greater independence and successfully return home or to the community.

What You'll Do
As a Community Transition Specialist, every day is different. You may:
? Meet with consumers and families to help them prepare for a successful transition back to community living.
? Visit nursing facilities and healthcare settings to support individuals planning to return home.
? Help consumers complete housing applications, MassHealth paperwork, and other benefit applications.
? Connect consumers with community services and support.
? Coordinate with healthcare providers, housing agencies, and community organizations.
? Assist consumers in overcoming barriers that may prevent a successful transition home.
? Maintain accurate documentation and case records.
? Prepare reports and update information in agency databases.

What We're Looking For
Required Qualifications
• High School Diploma or GED.
• One year of related work experience or a demonstrated interest in working with older adults, individuals with disabilities, or healthcare consumers.
• Strong communication and interpersonal skills.
• Excellent organizational skills and attention to detail.
• Ability to manage multiple priorities in a fast-paced environment.
• Proficiency in Microsoft Office applications.
• Valid driver's license.
• Access to a legally insured and registered motor vehicle.
• Ability to travel throughout ASNCM's service areaÂ
