We are currently seeking a compassionate, organized, and motivated Behavioral Health Home (BHH) Health Home Coordinator – Adults to join our team. Position Summary The BHH Health Home Coordinator works collaboratively within a multidisciplinary team to support adults with behavioral health needs through integrated, person-centered care coordination. This role promotes access to healthcare, supports physical and behavioral health integration, and connects clients to community resources and social support services. The coordinator plays a key role in reducing unnecessary hospitalizations and improving overall health outcomes by partnering closely with members, healthcare providers, and community organizations. What You’ll Do Provide coordinated, team-based behavioral and physical healthcare support Complete comprehensive face-to-face assessments and individualized care plans within required timelines Maintain accurate and timely documentation and case records Ensure client voice and choice are central to care planning using wraparound principles Collaborate with healthcare providers, hospitals, social service agencies, law enforcement, and community partners Connect clients to essential community resources including housing, transportation, financial assistance, healthcare, counseling, and wellness services Coordinate referrals for medically necessary services including OT/PT, dental care, hospice, home health, and behavioral health services Monitor care plan progress through telephonic and in-person support Participate in supervision, trainings, and team meetings Maintain HIPAA compliance and confidentiality standards Qualifications Education High School Diploma or GED required MHRT/C (Mental Health Rehabilitation Technician/Community) certification required Experience Minimum of 2 years of experience in the behavioral health field Skills & Qualities Excellent communication and customer service skills Strong organizational and multitasking abilities Ability to thrive in a fast-paced environment Sound judgment and problem-solving skills Team-oriented mindset Proficiency with Microsoft Office (Word, Excel, Outlook, PowerPoint) Reliable transportation and valid driver’s license required Commitment to professional growth and development Schedule Full-time, 40 hours/week Monday through Friday Flexibility to meet organizational and client needs required Work Environment This position works across multiple settings including: Office environments Community settings Client homes Physical Requirements Ability to lift up to 25 pounds Frequent sitting and computer use Walking, kneeling, squatting, stair climbing, and fine motor skills required Why Join Andwell?
