ESSENTIAL DUTIES and RESPONSIBILITIES
- Provide comprehensive assessment and diagnosis of Behavioral and Mental Health clients.
- Develop effective treatment plans, that include specific goals to be met and timeline for meeting goals
- Assist clients in achieving established self-management goals
- Evaluate crisis situations and apply appropriate interventions or referrals if needed
- Actively participate in meetings that support an integrated Medical/Mental Health care model to provide comprehensive care for clients
- Assist in the detection of āat riskā patients and development of plans to prevent further psychological or physical deterioration.
- Assist in the management of chronic disease by working with patients in self-management of chronic diseases
- Refer patients for other treatment when objective progress in patientās condition cannot be documented
- Provide trainings for staff and/or families on Behavioral Health issues as needed
- Must comply with HIPAA, HITECH, and Part II regulations.

EDUCATION and/or EXPERIENCEĀ
- Licensed Clinical Social Worker with current Kentucky license
- Must maintain compliance with licensure board continuing education requirements
- A minimum of 2 years of direct clinical experience
- Integrated medical care model experience preferredĀ

- Computer skills - knowledge of word processing software
- Excellent working knowledge of Behavioral medicine and evidence-based treatments for Mental Health and Behavioral Health conditions
- Ability to work through brief patient contacts as well as to make quick and accurate clinical assessments of Mental Health and Behavioral Health conditions
- Ability to communicate and work with an interdisciplinary team in a primary care centerĀ serving large numbers of low income and minority patients
- General knowledge of the clinical effects and side effects of prescription and illicit drugs used by Mental Health and Behavioral Health patients
- Ability to design and implement clinical pathways and protocols for treatment of selected chronic Mental Health and Behavioral Health conditions
SPECIAL KNOWLEDGE and SKILLS
This position requires effective oral and written communication skills, excellent interpersonal skills, and intermediate computer literacy.
LANGUAGE SKILLSĀ
Incumbent must possess the ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ā Incumbent must possess the ability to write reports, business correspondence, and procedure manuals; effectively present information and respond to questions from groups of managers, patients, other employees, and the general public.
MATHEMATICAL SKILLSĀ
Incumbent must possess the ability to work with mathematical concepts such as probability and statistical inference, and the ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.

Incumbent must possess the ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Incumbent must possess the ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.Ā
CERTIFICATES, LICENSES, REGISTRATIONSĀ
Must be licensed in the state of Kentucky as a Licensed Clinical Social Worker. Ā Must maintain mandatory continuing education requirements. Ā Current CPR certification is required and must be maintained throughout the duration of this position.Ā
PHYSICAL DEMANDS
While performing the duties of this job, the incumbent is regularly required to sit, talk, and hear. The incumbent is frequently to use hands and fingers to handle and feel; hands and arms to reach. The incumbent will also be required to walk, stand, stoop, kneel, crouch, crawl, climb, or balance. Ā The incumbent must occasionally lift and/or move up to 25 pounds. Specific vision abilities include close and distant vision within normal range, as well as the ability to adjust focus.

While performing the duties of this job, the incumbent is occasionally exposed to outside weather conditions. Ā The noise level in the work environment is usually moderate.
