Commonwealth Medical Services is seeking a compassionate and dedicated Family Medicine Physician to join our primary care team in Lima, Ohio. This position offers the opportunity to provide comprehensive outpatient care for patients of all ages while working within a collaborative healthcare environment focused on preventive medicine, chronic disease management, and continuity of care. The ideal candidate is committed to delivering high-quality, patient-centered care and building lasting relationships within the community.

Responsibilities

  • Provide comprehensive primary care services for pediatric, adult, and geriatric patients.
  • Diagnose and treat acute and chronic medical conditions.
  • Perform preventive health screenings and annual wellness examinations.
  • Develop individualized treatment plans and coordinate specialty referrals when appropriate.
  • Educate patients regarding disease prevention, medications, nutrition, and healthy lifestyles.
  • Maintain accurate and timely medical documentation.
  • Participate in quality improvement initiatives and clinical team meetings.
  • MD or DO degree from an accredited medical school.
  • Board Certified or Board Eligible in Family Medicine.
  • Eligible for unrestricted Ohio medical licensure.
