Are you passionate about creating meaningful experiences and improving the quality of life for seniors? Lincoln Meadows Care Center is seeking a dedicated and compassionate Activities Director to lead our Activity Department in a skilled nursing facility.

The Activities Director is responsible for planning, organizing, implementing, and evaluating a resident-centered activities program that supports the physical, mental, emotional, social, and psychosocial well-being of our residents.

What You'll Do

  • Plan, develop, organize, implement, and evaluate a comprehensive resident-centered activities program.
  • Develop monthly activity calendars that include group activities, resident outings, special events, and individualized or in-room activities.
  • Design activities based on each resident's interests, abilities, needs, and comprehensive assessment.
  • Encourage residents to participate in hobbies, crafts, reading, games, social activities, and other self-directed interests.
  • Provide appropriate materials and resources to support resident participation, including alternative formats when needed.
  • Participate in the development and implementation of activity care plans and resident assessments.
  • Document activity participation and progress notes accurately and thoroughly.
  • Maintain compliance with current federal, state, local, and facility regulations and professional standards.
  • Participate in resident and family meetings, Resident Council, Family Council, Quality Assurance/Performance Improvement activities, department head meetings, and other facility committees.
  • Work collaboratively with nursing, therapy, dietary, social services, and other departments to support resident-centered care.
  • Assist with discharge planning and transportation arrangements when needed, including outings, appointments, and discharge.
  • Communicate effectively with residents, families, staff members, community partners, government representatives, and other stakeholders.
  • Participate in facility surveys and assist with plans of correction related to activity department deficiencies.
  • Stay current with regulatory requirements and best practices related to activities programming in long-term care.
  • Assist with the supervision and management of Activity Department staff.
  • Promote a positive, welcoming, and engaging environment where residents feel valued and included.

What We're Looking For

Required:

  • High school diploma or equivalent.
  • Activity Director certification.
  • Ability to read and understand policies, procedures, and technical information.
  • Strong communication and interpersonal skills.
  • Ability to organize activities, manage schedules, and meet deadlines.
  • Ability to solve practical problems and make sound decisions.
  • Ability to work effectively with residents, families, staff, and leadership.

Preferred:

  • At least one year of experience working in a long-term care or skilled nursing facility.
  • Experience developing and implementing resident-centered activities programs.

Physical Requirements

This position requires frequent standing and walking, reaching, pushing and pulling, talking and hearing. The position may also require frequent lifting of up to 50 pounds and occasional climbing, balancing, stooping, kneeling, crouching, or crawling. Appropriate vision abilities, including close, distance, peripheral, color, and depth perception, are required.
