Monday  - Friday plus weekend on-call rotation (limited remote phone support as needed)

The Clinical Admissions Liaison drives occupancy growth and census performance by managing referrals, evaluating admission appropriateness, and building strong relationships with hospitals and community partners. This role serves as a key link between referral sources and the Health Center, ensuring timely admissions, smooth resident transitions, and exceptional customer service throughout the admission process.
Key Responsibilities
- Achieve occupancy and census goals.
- Review referrals and clinical documentation to determine admission eligibility.
- Conduct clinical assessments and coordinate admissions with internal teams.
- Ensure smooth transitions between hospitals, skilled nursing, and other levels of care.
- Triage hospitalized residents and coordinate appropriate placement or return to care.
- Build and maintain relationships with hospital case managers, discharge planners, physicians, social workers, and other referral sources.
- Conduct hospital visits, bedside assessments, rounds, and community outreach activities.
- Track referrals, admissions, and professional contacts within CRM/EHR systems.
- Collaborate with admissions, clinical, and operations teams on referral and census management.
- Support outreach events, marketing initiatives, and community engagement efforts.
- Assist with training and guidance of Admissions Coordinators.
- Serve as backup to the Admissions Director and provide coverage as needed.
- Participate in on-call, weekend, and emergency response responsibilities.

Qualifications
- Active clinical license required: RN, LPN, PT, PTA, OT, COTA, ST, or Clinical Social Worker.
- Bachelor's degree preferred.
- 3-5 years of clinical experience in acute care, post-acute care, or senior living.
- Admissions, liaison, business development, or census management experience preferred.
- Strong clinical assessment, critical thinking, and decision-making skills.
- Proven ability to build referral relationships and achieve occupancy goals.
- Experience with EMR/CRM systems; PointClickCare experience preferred.
- Excellent communication, organization, and customer service skills.
- Proficiency with Microsoft Office.
- Valid driver's license and reliable transportation.
- Ability to work independently, manage multiple priorities, and maintain a flexible schedule.
