You’ll also lead and support Physical Therapist Assistants (PTAs), ensuring that care is delivered safely, effectively, and aligned with each patient’s goals. Working closely with an interdisciplinary team, you’ll continuously adjust treatment plans to drive the best possible outcomes.
In this role, you will:
- Conduct comprehensive evaluations and establish clinical diagnoses to guide care
- Develop and implement individualized treatment programs focused on real progress and function
- Reassess patients and modify care plans to maximize recovery and results
- Apply infection control and safety standards in all patient interactions
- Maintain accurate, timely, and compliant documentation
- Communicate and collaborate with patients, families, and care teams
- Supervise PTAs to ensure quality and continuity of care

Requirements
Successful completion of accredited physical therapy program and state licensure exam.
Valid Physical Therapy license
1 year of home health experience preferred
CPR certification
Valid Driver’s license
Valid Auto Insurance
HomeCare HomeBase experience preferred
Physical Demands
Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
Must be able to lift, carry, pull, or push up to 100 lbs..

Pay-Day
The Company operates a bi-weekly payroll process, paid on Tuesdays (10 days in arrears), normally 26 times annually. If one of the paydays falls on a Tuesday, Company Holiday, the

Working Hours
To meet the needs of our patients and clients, employee work hours may vary and will be set by each employee’s supervisor, manager, and operating license documentation.

are 8:00 am to 5:00 pm (CST).
Mileage
The Company recognizes the use of personal vehicles by personnel for business purposes as part of their employment. The company will reimburse the eligible employee $0.50 per
mile to cover personal care use, auto insurance, depreciation, and gas. Each employee who is eligible for mileage reimbursement from the Company will be required to track and report mileage
accurately and timely each pay period.Â
