Experienced Social Worker - Transfer of Care Hub and Hospital bases - Liverpool

Liquid Personnel is currently recruiting for experienced Social Workers to join the Transfer of Care Hub and Hospital Bases at Liverpool City.

What will your responsibilities be?
In this role, you will assess individuals in hospitals for care packages or care home placements under the Care Act, conduct Mental Capacity Assessments for care and support, and collaborate with multidisciplinary teams to create care plans. You will engage in carers conversations and facilitate safeguarding discussions, ensuring a personal approach throughout.

Qualifications and Experiences:
To succeed in this role, you must have Social Work England registration and be eligible to work in the UK. You should hold a qualification in Social Work at degree level (or equivalent) and have post-qualified experience. A valid UK driving license is also required.
