This is a full time position for MLIH's mobile unit. Requires regular and routine travel on the mobile unit.
Position Purpose
The Licensed Practical Nurse (LPN) provides nursing care and clinical support to patients served through the organization’s mobile health unit. The LPN works as part of the mobile health team to deliver safe, compassionate, and efficient patient care in community-based settings. This position is responsible for supporting daily clinical operations, completing patient assessments and nursing tasks within the LPN scope of practice, maintaining accurate and timely documentation in the agency’s Electronic Health Record (EHR) system, and scheduling patients for services and follow-up visits. The LPN will also assist with patient flow, mobile unit readiness, community outreach activities, and coordination with clinical staff and community partners.
Essential Job Functions
Patient Care and Clinical Support
- Provide nursing care to patients within the scope of practice for a Licensed Practical Nurse and in accordance with organizational policies, procedures, and applicable laws and regulations.
- Obtain and document patient vital signs, health histories, medication information, and other relevant clinical information as directed.
- Assist providers and other clinical staff with examinations, treatments, procedures, and other patient care activities.
- Identify and communicate changes in a patient’s condition or other clinical concerns to the appropriate provider or supervisor.
- Administer medications and treatments as ordered and permitted within the LPN’s scope of practice.
- Assist with specimen collection, point-of-care testing, screenings, and other clinical services provided through the mobile unit.
- Provide patient education and reinforce instructions provided by providers and other members of the healthcare team.
- Support preventive care, health education, and wellness activities conducted through the mobile unit.
- Maintain patient privacy, dignity, and confidentiality at all times.
EHR Documentation and Patient Scheduling
- Accurately document patient encounters, nursing services, assessments, interventions, and other required information in the agency’s EHR system.
- Ensure clinical documentation is completed accurately, timely, and in accordance with agency standards.
- Coordinate with clinical and administrative staff regarding patient appointments, referrals, follow-up needs, and continuity of care.
Mobile Unit Operations
- Assist with preparing the mobile health unit for scheduled service days and ensuring necessary clinical supplies and equipment are available.
- Help maintain a clean, safe, organized, and professional clinical environment within the mobile unit.
- Check clinical equipment and supplies as assigned and report missing, damaged, expired, or malfunctioning items to the appropriate supervisor.
- Assist with stocking and maintaining medical supplies and equipment.
- Ensure patient flow and that patients are appropriately directed through the mobile unit’s registration, clinical, and discharge processes.
- Assist with setup and breakdown of the mobile unit at community service locations as needed.
- Follow established procedures for infection prevention, environmental safety, emergency preparedness, and workplace safety.
- Promptly report clinical, equipment, safety, or operational concerns to the appropriate supervisor.
Community and Patient Relations
- Assist with greeting and communicating with patients at mobile unit locations.
- Communicate professionally with community partners and other individuals involved in supporting mobile unit operations.
- Assist with outreach, health education, screenings, and community-based activities as assigned.
Minimum Requirements
- Current and unrestricted Licensed Practical Nurse (LPN) license in West Virginia.
- Graduate of an accredited practical nursing program.
- Current CPR/BLS certification or ability to obtain certification within the required timeframe.
- Previous nursing or healthcare experience preferred.
- Experience working with an Electronic Health Record (EHR) system preferred.
- Strong communication, organizational, and interpersonal skills.
- Ability to travel to various community locations as required by the mobile health unit schedule.
Working Conditions:
This position may involve being in the mobile unit and/or community at least 80% of the time. When in the agency, this position is maintained in a controlled climate. No more than 20% of employee’s time is spent in standing and moving about the agency. This employee will only be required to do lifting of less than 50 pounds no more than 10% of the time.
