Lone Tree Post Acute is looking for a compassionate and motivated Certified Occupational Therapy Assistant (COTA) to join our in-house rehabilitation team. As part of our 99-bed skilled nursing facility, you'll work alongside experienced therapists and healthcare professionals to help residents regain independence, improve daily function, and enhance their quality of life.

What You'll Do
Provide skilled occupational therapy treatments under the supervision of the Occupational Therapist
Help residents improve functional abilities, independence, and overall quality of life
Collaborate with therapists, nursing staff, physicians, and the interdisciplinary care team to achieve resident goals
Educate residents, families, and caregivers on therapy techniques, adaptive equipment, and home safety
Assist with discharge planning to support safe transitions home
Maintain accurate, timely clinical documentation in compliance with regulatory standards
Foster a positive and encouraging therapy environment that motivates residents throughout their rehabilitation journey
What We're Looking For
Current, active California Certified Occupational Therapy Assistant (COTA) license
Excellent communication and interpersonal skills
Strong teamwork and organizational abilities
Commitment to compassionate, resident-centered care
