Lone Tree Post Acute is looking for a compassionate and motivated Certified Occupational Therapy Assistant (COTA) to join our in-house rehabilitation team. As part of our 99-bed skilled nursing facility, you'll work alongside experienced therapists and healthcare professionals to help residents regain independence, improve daily function, and enhance their quality of life.

What You'll Do

  • Provide skilled occupational therapy treatments under the supervision of the Occupational Therapist

  • Help residents improve functional abilities, independence, and overall quality of life

  • Collaborate with therapists, nursing staff, physicians, and the interdisciplinary care team to achieve resident goals

  • Educate residents, families, and caregivers on therapy techniques, adaptive equipment, and home safety

  • Assist with discharge planning to support safe transitions home

  • Maintain accurate, timely clinical documentation in compliance with regulatory standards

  • Foster a positive and encouraging therapy environment that motivates residents throughout their rehabilitation journey

What We're Looking For

  • Current, active California Certified Occupational Therapy Assistant (COTA) license

  • Excellent communication and interpersonal skills

  • Strong teamwork and organizational abilities

  • Commitment to compassionate, resident-centered care
