Job Summary

Supports the Director of Patient Care Services and clinical team to ensure high-quality patient care, efficient staff coordination, and compliance with all regulatory requirements in a home health setting.

Key Responsibilities

  • Coordinate clinician schedules and patient assignments to maximize efficiency and coverage
  • Monitor productivity and ensure clinicians meet visit expectations
  • Support onboarding, training, and ongoing staff development
  • Promote strong interdisciplinary communication and teamwork
  • Ensure compliance with state, federal, and Medicare regulations
  • Conduct chart audits and support quality assurance initiatives
  • Communicate with physicians, patients, families, and pharmacies
  • Assist field staff with orders, lab follow-ups, and care coordination
  • Maintain accurate patient and medication records

Qualifications

Required:

  • Active clinical license
  • 4+ years of clinical experience (home health preferred)
  • Knowledge of healthcare regulations and Microsoft Office
  • Strong organizational, communication, and decision-making skill
  • Bachelor’s degree in a clinical field
  • 2+ years of supervisory experience
