Cura Health is seeking a compassionate and organized Social Services Designee to join our team. In this role, you will provide essential support by assisting with resident social services and care planning activities. You will collaborate with social workers, healthcare providers, residents, and their families to ensure residents’ psychosocial needs are met and resources are effectively coordinated. The Social Services Designee plays a critical role in promoting the overall well-being and quality of life of residents within our healthcare facility. Key Responsibilities
  • Assist in assessing residents' social, emotional, and psychological needs as part of the interdisciplinary care team.
  • Coordinate and facilitate communication between residents, families, and healthcare providers regarding care plans and available resources.
  • Support discharge planning processes and link residents with community resources.
  • Maintain accurate and confidential resident records in compliance with organizational policies and regulations.
  • Advocate for residents’ rights and ensure their preferences and needs are respected.
  • Participate in team meetings and contribute to multidisciplinary care planning.
  • Provide counseling and crisis intervention as appropriate under supervision.
  • Previous experience in social services, healthcare, or related fields preferred.
  • Strong communication, interpersonal, and organizational skills.
  • Ability to work empathetically with elderly or vulnerable populations.
  • Knowledge of community resources and healthcare systems.
  • Ability to maintain confidentiality and handle sensitive information professionally.
  • Effective problem-solving and crisis intervention skills.
  • Ability to work collaboratively as part of an interdisciplinary team.
  • Relevant educational background in social work, psychology, or related fields is a plus.
