Speech Language Pathologist - SLP | Legacy Lifecare VNA/Hospice
$110/per assessment | $75/routine therapy sessions | $45/hr for administrative work

Delivering Solutions that Matter:

  • Evaluate patients for speech and language disorders, initiate assessments upon referral, and develop individualized plans of care in collaboration with the physician and multidisciplinary care team.
  • Provide rehabilitative speech-language therapy services, carry out appropriate interventions, and educate patients, families, and caregivers on treatment procedures and supportive activities.
  • Monitor, document, and report patient progress, responses to therapy, and changes in condition using the agency's Problem Oriented Record (POR) and required clinical documentation systems.
  • Coordinate care with healthcare team members, consult with supervisors, and participate in case conferences, in-services, and ongoing professional development.
  • Support patients and families in adapting to physical, mental, and social limitations by providing education, encouragement, and resources to promote independence and continuity of care.
  • Maintain compliance with agency policies, patient rights, confidentiality, infection control, safety standards, and emergency preparedness while managing workload, scheduling, and priorities effectively.

Qualifications for Success:

  • Active, good-standing MA Speech-Language Pathologist license
  • 1-3 years of experience in geriatrics, long-term care, or short-term rehab
  • Ability to communicate fluently in English - both verbally and written
