Home Health Case Manager - RN | Home Care
$42.00/hour - $45.00/hour | Guaranteed full hourly pay for all scheduled hours!

We are seeking a compassionate and skilled RN Case Manager to provide high‑quality primary nursing care to patients in their homes and Assisted Living communities. This role focuses on assessment, care planning, patient education, and coordination of services under a physician’s plan of treatment.

What You'll Do:  

  • Complete initial and ongoing patient assessments and develop individualized plans of care
  • Provide skilled nursing services, including preventative and rehabilitative care
  • Monitor patient progress and respond promptly to changes in condition
  • Educate and support patients and families
  • Coordinate care with physicians, caregivers, and home health team members
  • Maintain accurate and timely clinical documentation
  • Supervise and support home health aides as needed

What You'll Need:  

  • Active unencumbered MA Registered Nurse (RN) license 
  • Graduate of an accredited school of nursing
  • Minimum 1 year of RN nursing experience
  • CPR certification 
  • IV therapy experience a plus
