Position Title: Wellness CoordinatorÂ
Classification: NonexemptÂ
Reports To: Administrator/RSDÂ
Pay Range: $21.00 - $23.00 /HourÂ
Summary/Objective:Â
The primary purpose of the Wellness Coordinator is to direct the day to day medical and medication related services provided to residents in accordance with current federal, state and local regulations and following SHO policies and procedures.Â
Essential Functions:Â
- Participate in the quality management program of the facility as directed by the resident services coodinator or administratorÂ
- Provide guidance to QMAPs and/or PCPs regarding medication related issuesÂ
- Orients and trains new QMAP and PCP staff on medication room procedures and documentationÂ
- Monitor staff performance in medication room procedures and documentation; notify Resident Services Director of concerns related to staff performance in medication room procedures and documentationÂ
- Order/obtain medications and PCP supplies as needed to fulfill physician’s ordersÂ
- Prepare medication administration records and other medical file contents for new residentsÂ
- Discuss medication room procedures with new residents, including appointment management,Â
medication procurement and physician’s ordersÂ
- Update all resident medication administration records as needed for physician order changesÂ
- Update resident information record (face sheets) and medical chart information as neededÂ
- Complete documentation and medication processing for resident transfers and dischargesÂ
- Document in communication log, resident chronological notes, flow sheets, etc. in an informative and descriptive manner according to SHO policyÂ
- Evaluate content of record to assure complete documentation by QMAPs and PCPsÂ
- Assess residents for changes in condition and implement proper interventionsÂ
- May notify family members, and/or case manager of change of conditionÂ
- Communicate with physicians, diagnostic services, hospitals and other outside providers as neededÂ
- Fill medication reminder boxes according to policiesÂ
- Assist residents to schedule medical appointments and transportation as neededÂ
- Prepare paperwork to send to physician appointmentsÂ
- May work as a QMAP or PCP to cover shifts as neededÂ
- Obtains a physician update for each resident on a semiannual basis.Â
- Respect resident privacy and maintain confidentiality of resident informationÂ
- Observe and follow all safety protocols and fire and emergency plans; complete accident/incident reports as neededÂ
- Always respect resident rights, and report concerns to a supervisorÂ
- Other duties as assignedÂ
Work Environment:Â
- Sitting, standing and walking throughout the day with intermittent drivingÂ
- Using office equipment such as computer, mouse, keyboard, printerÂ
- Must be able to lift up to 30 lbs. comfortablyÂ
- Comfortable using and climbing on step stoolsÂ
- Certain activities may require repetitive movements including climbing, crawling, crouching, pulling, pushingÂ
- May be subject to exposure to infectious waste, diseases, conditions including HIV and Hepatitis B viruses.Â

- Strong organizational and interpersonal skills; attention to detailÂ
- Intermediate computer knowledge and typing skills; working knowledge of Microsoft Office SuiteÂ
- Respect for privacy and ability to maintain confidentiality of resident care informationÂ
- Enjoys community involvement, resident interaction, and public representation of the organizationÂ
- Willingness to be a team player and provide support to all departments as needed with the intent to provide the best care to our residentsÂ
- Bilingual a plusÂ
Education and Experience:Â
- High school diploma or equivalent requiredÂ
- Prior experience working in healthcare environment or working with an elderly population preferredÂ
- Training and designation as Qualified Medication Administration Person (QMAP) through program approved by the State of Colorado Department of Health requiredÂ
- Read, write, speak and understand the English languageÂ
- Demonstrated working knowledge of Microsoft Office Suite including Word, Excel, OutlookÂ
Expected Hours of Work of a Wellness Coordinator:Â
This role may be full-time or part-time. Schedule is generally Monday through Friday. May be called during off hours to respond to emergency or unusual situations.Â
