Physical Therapist - PRN

Position Summary
The Physical Therapist (PT) will provide skilled home health services to patients, focusing on improving functional independence, safety, and overall quality of life. This role involves patient assessments, individualized treatment planning, and ongoing collaboration with the care team.

Key Responsibilities

• Perform comprehensive patient assessments and evaluations
• Develop and implement individualized therapy treatment plans
• Provide occupational therapy services in the patient’s home
• Document patient progress, outcomes, and medical histories accurately
• Communicate effectively with patients, families, and interdisciplinary team members
• Ensure compliance with all clinical and regulatory standards

Qualifications

• Active Physical Therapist (PT) license in the state of Colorado
• Reliable transportation and valid auto insurance
• Strong clinical judgment and patient-centered care approach
• Excellent communication and documentation skills
• Prior home health experience preferred (training available for qualified candidates)
