Job DetailsJob Location: Anchor Lodge - Lorain, OH 44053Sprenger Health Care is a third generation, leading developer, owner and operator of senior living communities and services in Ohio and South Carolina. Through our 12-nursing home, 10 assisted living, and 8 independent living communities encompassing nearly 2,000 beds, we provide exceptional health care services, including; long-term nursing care, assisted and independent living, memory care, hospice, home care, respite care, and short-term post-acute rehabilitation. For more information about Sprenger Health Care, please visit www.sprengerhealthcare.com. Qualifications: Bachelor of Science Degree in Physical Therapy from and accredited program. Successful completion of National Certification Examination for Registered Physical Therapist. Assumes responsibility for ongoing continuing education and professional development. Current state licensure or license eligible. Essential Job Functions: Provides physical therapy services to residents and initiates referrals when appropriate and in accordance with Physical Therapy Department Procedures. Conducts screenings of residents at regular intervals to determine need for intervention/treatment. Evaluates residents to obtain data necessary for treatment planning and implementation and conducts specialized evaluations as indicated. Coordinates treatment plan with resident, family and other team members. Implements and conducts treatment as outlined in treatment plan.
