• Monday - Friday, 8 am - 5 pm
• 20 - 25 patients per day -- 10 patients per 4-hour session
• Community Health Center -- outpatient family practice setting
• CA License Required
• Support staff includes 1 NP, 1 RN, 1 MA on site
• EMR: Epic
• Experience in chronic pain, addiction medicine, behavioral health required
• Rural setting with one other family practice physician
