POSITION SUMMARYÂ
Reports to the Department Operations Supervisor under the direction of the Program Director. Provides patient supervision and staff/unit support activities as assigned.

CATEGORIES OF DUTIESÂ
1. Provides clerical support to counseling, case management staff, e.g., filing, make copies of forms, letter writing, answering phones, breakdown of discharge charts, update white boards with patient assignments, and chaff auditing, etc.Â
2. Provides transportation services utilizing company vehicles, including patient transportation, donations, pick-ups, and agency errands, etc.Â
3. Patient escorting and monitoring.
 4. Attends in-service training and educational workshops.Â
5. Attends all required staff meetings.Â
6. Cooperates and communicates fully with other staff members regarding patient behavior, progress, and activities, etc.Â
7. Conducts self in a professional manner in areas of dress, language, attitude, and behavior.Â
8. Exhibits full knowledge and support of the philosophy and objectives of the Agency.Â
9. Fully understands and maintains policies regarding professional ethics, including appropriate boundaries and patient confidentiality.Â
10.Responsible for inspecting and ensuring proper maintenance of the company vehicle used. Fuels vehicle as needed.Â
11.Other duties as assigned by Supervisor, Director and/or Administration.
LICENSES/CERTIFICATES
Valid  Driver’s License and proof of auto insurance.
EDUCATION/EXPERIENCE
 • High School Diploma or GEDÂ
• Proficiency using GPS or navigation softwareÂ
• Excellent driving skills and knowledge of traffic lawsÂ
• Minimum of 4 years driving experience, clean driving record with no major violations
