Competitive Wages & Annual Merit Increase Monday-Friday 7:30am-4pm This position has multiple responsibilities: coordinating shower operations for unhoused people,providing administrative support (including basic care coordination functions) to the Homeless Services Program (including its mobile clinic, satellite clinic, and street medicine programs); and EITHER delivering pharmaceutical supplies support to unhoused patients OR providing comprehensive insurance enrollment and benefit navigation services to patients served by the Homeless Programs. Essential Functions:  Coordinates, monitors, and maintains shower schedules. Orients new shower clients with the following: Connection to appropriate staff to ensure registration and insurance enrollment Basic assessment for linkage to other medical and dental services Shower Program rules Monitors client behavior and ensures compliance with Shower Program rules for new and existing clients. Distributes disposable towels, toiletries, and/or donations (if applicable) to each shower client. Maintains order and assists facilities personnel in keeping all assigned areas clean and free of safety hazards. Performs routine shower checks. Stores and organizes donations. Completes incident reports in a timely manner. Contacts security guard and on-site designated supervisor for any security concerns within or immediately outside the facility. Links Homeless Program patients to a medical & dental home as well as community resources and services to support their overall wellness and recovery. Corresponds directly with care providers to develop care plans for patients & assists with specialty medical & dental care referrals (including follow-up). Pharmaceutical Delivery Support for Unhoused People Delivers medications and pharmaceutical supplies (excluding narcotics) to homeless services patients (including at the satellite clinic, mobile clinic sites, and other approved locations). Ensures safe, secure, and confidential transportation of medications in compliance with all applicable regulations and organizational guidelines. Follows assigned routes and delivery schedules as directed by the Pharmacy Supervisor. Loads, unloads, prepares, inspects, and operates vehicles used for medication delivery. Completes delivery logs, documentation, and proof of pharmaceutical delivery as required. Patient Navigation Supports patients with insurance status queries and enrollment: assists with primary care switches, insurance applications, and referrals. Provides in-field support as needed: visits partner sites to support patients with registration forms and insurance questions prior to provider visits. Position Requirements: High school diploma or equivalent Bi-Lingual in Spanish a plus Minimum 1-3 years of program administrative experience (non-profit community health experience a plus) Excellent customer service skills Committed to working with the homeless and disconnected population; case management or outreach experience a plus Must be flexible and able to multi-task. Ability to maintain confidentiality in all aspects of the work environment. Basic computer knowledge; ability to use computers, tablets, cellphone for record keeping Attention to detail and accuracy in medication delivery  Current California license and state-required insurance when using personal vehicle on Clinic business and a driving record acceptable to the organization and/or insurance carrier.   Compensation Disclaimer Actual salary offers are considered by various factors, including budget, experience, skills, education, licensure and certifications, and other business considerations.
