What you bring to SBHG (Qualifications):Ā 

EducationĀ 

  • Facilitator II: Master's degree in a Mental Health, Human Services, or closely related fieldĀ 

  • Facilitator I: Bachelor's degree in Mental Health, Human Services, or closely related fieldĀ 

ExperienceĀ 

  • Facilitator II: One (1) year of experience in a mental health field, and direct service experience with severely emotionally disturbed children, adolescents and families; experience with high-risk children and families.Ā 

  • Facilitator I: Two (2) years of experience in a mental health field, and direct service experience with severely emotionally disturbed children, adolescents and families; experience with high-risk children and families.Ā 

Ā 

Licensure & Certification:Ā 

  • Valid California Driver’s License and maintenance of a driving record that meets the company driver eligibility policyĀ 

  • CANS Certification within 30-days of hireĀ 

  • Completion of Statewide Standardized Child and Family Team (CFT) Facilitation Training within one (1) year of hireĀ

How you will make a difference:Ā 

This position has primary responsibility for coordinating services across various programs, including engagement, assessment, Child and Family Team (CFT) formation and implementation, and development of Safety Crises Plan and Strengths Summary. Facilitators provide a broad range of case management services meant to stabilize and reduce involuntary hospitalization and/or placement disruption. When indicated as a need by the youth/adult/family, the Facilitator may provide mental health services such as, individual rehabilitation, collateral intervention, and coaching. He/she acts as a liaison with external agencies and convenes the Child and Family Team. Facilitators take the lead to ensure the team consists of family members, County representatives, the client, and others selected by the family. The Facilitator works to ensure all team members are active participants in the CFT Process.Ā He/SheĀ works with family to generate Mission statement and list of underlying needs, while assisting child/family and team to identify and prioritize needs. Collaborates with team members to ensure adherence to program process and principles, as well as, TEAMMATES philosophy of ā€œWhatever It Takes.ā€Ā

  • Population Served:Ā Children, Youth, and Families battling trauma, mental illness, neglect, or substance abuse |Ā 

  • Facility & Setting:Ā Multidisciplinary program sites serving SPA 1,2,3,4,6,7,8Ā Ā Ā Ā 

  • Clinical Services:Ā Trauma-informed care, mental health treatment, behavioral interventions , and parental skill enhancementĀ 

  • Length of Stay:Ā Based on need, average stay is 18 monthsĀ 

  • Skills & Transition:Ā Ā Ā 

    • Strengths-based empowerment, resilience building, and long-term recovery/re-integration supportĀ Ā Ā 

    • Family support, skill development, advocacy, and comprehensive community resourcesĀ Ā
