Direct Support Professional - Santa Clarita - Part-time (3pm - 11pm Thursday - Sunday)
- Maintaining records in the home according to MLF policy. (I.E., daily notes, behavior sheets, medical forms, etc.)
- Assist the individual being served, ensuring they remain safe in their homes as well as while in the community.
- Delivering individualized training and care for consumers in compliance with healthcare professionals’ instructions and other specific guidelines and documentation of the same.
- Facilitate consumers inclusion in their community by having experiences and activities that are in the community and documenting such activities.
- Work with a servant’s heart and aid where/when needed.
- Treat the individuals being served with dignity and respect.
- Advancing consumers decision-making, productivity, and participation in a range of activities.
- Promoting independence in daily activities, such as work, learning, and leisure.
- Ensure the highest quality of care/service for individuals being served.
- Support individuals with activities of daily living (I.E., personal hygiene, dressing, preparing meals etc.) according to their ISP’s/IPP’s and skill level.
- Implementing goals defined in the ISP/IPP for each individual.
- Assist individuals with daily household duties (I.E., laundry, sweeping, mopping, etc.)
- Assist individuals with medication administration according to MLF medication protocol and documenting the same.
- Assist in integrating individuals into the community.
- Assist with appointments as needed. (I.E., doctor, dentist, therapy, etc.) and ensure all applicable documentation is filled out correctly.
- Ensure clock in and out is done daily without need for correction.
- Perform other duties as assigned.
- Promoting person centered facilitation of services at all times.
