SUMMARY 

The Clinical Pharmacist assumes and carries clinical pharmacy services and fulfills day to day operational functions in the Pharmacy. Responsible for coordinating daily clinical and operational activities of OIC pharmacy programs, in accordance to OIC policies and procedures and requirements of State and Federal regulatory agencies.  Supervises pharmacy technicians and oversees the clinical pharmacy service programs. 

 

The pharmacist is responsible for monitoring drug therapy, providing drug information, consulting with medical, nursing, and allied healthcare professionals to deliver high quality, cost effective pharmaceutical services.  Assumes responsibility as a preceptor to pharmacy students.

 

Provides support to pharmacy department team to formulate and implement policies and plans to meet the OIC’s short-and long-term objectives.

 

Coordinates with various departments such as HTC Clinic and other allied health providers to fulfill duties and functions and provide patient care support activities.

 

Assumes full or partial responsibility for plans and projects designed to support the OIC’s mission and vision.

 

DUTIES AND RESPONSIBILITIES 

  • Assists the Chief Pharmacist in developing implementing and administering plans and programs for short and long range goals and objectives.
  • Coordinates and supervises the day to day department activities in accordance with established OIC policies, procedures, accepted standard practice, federal and state regulations.
  • Ensures compliance I areas of responsibility required department policies and procedures, State Board of Pharmacy and Federal regulations and Title 24.
  • Serves as a liaison between the departments of pharmacy, nursing, medical and allied health staff.
  • Selects, compounds, dispenses and prices pharmaceutical prescriptions as required to fulfill physician’s written, oral or e-RX orders.
  • Receives new prescriptions orally from a prescriber or other person authorized by law.
  • Processes and accurately interprets all prescriptions in a timely manner.
  • Reviews all orders for appropriateness and accuracy.
  • Performs the drug distribution procedures necessary to ensure-five rights. (right patient, right drug, right dose, right route and right time)
  • Knowledgeable and maintain required competency for filling and handing prescription for blood clotting factor products as well as any prescriptions for drugs that treat co-morbidities in hemophilia patients per California State Board of Pharmacy and California DHCS Blood clotting contract requirements.
  • Supervision and responsible for all pharmacy technicians activities to insure that all such activities are performed completely, safely and without risk harm to patients.
  • Performs clinical pharmacy functions accurately and in a timely manner, including an assessment of all aspects of medication related therapy necessary to provide for safety, efficacy, and cost-effectiveness off drug use.
  • Provide clinical activities to optimize pharmaceutical care including but not limited to: the regulation of drug therapy, provision of drug information, preventions of drug interactions and adverse drug events, screening for drug allergies, evaluating and monitoring drug therapy.
  • Recognizes and addresses the age-related needs of the specific population served.
  • Provides accurate dosing information to all practitioners and counsels patients/family with regards to the proper use of each medication dispensed in accordance with state and federal regulations
  • Participates in OIC pharmacy qualify improvement projects per ISMP best practices.
  • Performs all functions which require professional judgement and any duties which federal or state regulation authorizes only a pharmacist to perform.
  • Be Knowledgeable of/and help with all billing requirements for all prescriptions especially for anti-hemophilic factor.
  • Receives all incoming pharmaceutical supplies from wholesaler or manufacturer and familiar with inventory maintenance, reconciliation and reports.
  • Checks and records all incoming and outgoing controlled substances; keeps current perpetual inventory to conform with government regulations making sure that controlled substances record is accurate at all time.
  • Maintains perpetual inventory of anti-hemophilic factor and drugs to assure accuracy of inventory levels.
  • Perform monthly inspection to remove outdates products from shelves.
  • Insures compliance with all policies regarding security, patient safety, equipment maintenance and patient confidentiality according to California State Board of Pharmacy, DEA and other regulatory agencies.
  • Knowledgeable and competent about HRSA 340B program and ensures compliance with 340B statue and regulations.
  • Keeps working area in a clean and orderly manner.
  • Utilizes good communication skills in handling all issues.
  • Provides in-service programs to physicians, residents, nurses, pharmacists and other allied healthcare professionals.
  • Acts as a preceptor to pharmacy students and pharmacy technician students.
  • Performs any other related duties assigned.

 

PERFORMANCE STANDARDS AND SUCCESS INDICATORS

  • Provides excellent patient care with no or minimal errors while processing medication orders (including near misses). This includes:
  • Assessment of allergies and previous adverse reactions before dispensing any prescription
  • Providing drug counseling to patients and their families with all new prescriptions and making sure that no further counseling is needed for refills
  • Demonstrates knowledge and competency in proper handling and billing of all Hemophilia related orders and problems.
  • Makes sure there are no outdates in the assigned area and in the Pharmacy as a whole
  • Provides cost effective drug therapy while maintaining the safety and accuracy of ordered drug therapy
  • Accurate and complete billing information for all prescriptions (especially Factor), making sure authorizations, medical necessity or any other requirements for correct reimbursement are done.

 

QUALIFICATIONS

  • Graduated from an accredited school of pharmacy recognized by the California State Board of Pharmacy with a Doctorate of Pharmacy.
  • Possesses a current license as a registered pharmacist issued by the California Board of Pharmacy.
  • Must have effective communication skills. Must be able to read, analyze and interpret pharmacy related journals Must have the ability to respond to common inquires or complaints from customers, regulatory agencies or members of the OIC and request help from the Director of Pharmacy as needed.
  • Must be willing to work any day coverage is needed during pharmacy open hours and is willing to work overtime and emergency callbacks, if required.

 

Complexity:

Ability to perform highly detailed work with sustained attention and ware while providing/obtaining information on numerous inquiries is paramount to success in this position.

 

 Temporary assignment; M-F 7:30am-4:00pm
