Position Title:     Licensed Clinical Social Worker (LCSW)                                                       

Summary:

Under direction of the Behavioral Health Manager, the LCSW is responsible for providing mental health and substance abuse services to Arroyo Vista Family Health Center patients.  

DUTIES AND RESPONSIBILITIES: 

  1. Responsible for assessing patients’ psychosocial status as it relates to substance abuse and mental health, communicates with primary care providers to report patient progress, needs, concerns, and potential and actual clinical issues.
  2. Responsible for providing individual and group counseling to patients in a strengths-based, culturally sensitive linguistically appropriate and compassionate manner. 
  3. Responsible for providing brief therapy, implementing appropriate evidenced based practices and interventions in client sessions.
  4. Responsible for establishing strong rapport with patients, which expresses concern for their well-being.
  5. Responsible for preparing and maintaining all pertinent records, statistics, and progress notes in a time sensitive manner.
  6. Responsible for working closely with all members of the health care team in development of treatment plans, based on counseling assessment and social work interventions.
  7. Responsible for partnering with the Behavioral Health Case Manager and ECM Program Team to refer patients to appropriate specialty services and community resources.
  8. Responsible for serving as ECM Clinical Consultant, as per ECM policy and procedures manual and contractual requirements.
  9. Responsible for assisting with ongoing clinical training, support, workshops and in-services to Agency staff as needed.
  10. Responsible for actively participating in the Quality Management Program.
  11. Responsible for following all Agency safety and health standards, regulations, procedures, policies, and practices.
  12. Responsible for actively participating in the Agency’s Improving Organization Program to include TJC and Management of the Environment of Care Program. 
  13. Responsible for maintaining Licensed Clinical Social Worker (LCSW) credential in good standing with the California Board of Behavioral Sciences, including completing 36 hours of continuing education every two years. 

REQUIREMENTS:

  1. Masters’ Degree in Social Work
  2. LCSW credential in good standing with the California Board of Behavioral Sciences.
  3. Minimum three (3) years’ LCSW experience
  4. Effective written and verbal communication skills and ability to write clear concise reports
  5. Knowledge of and experience with providing substance abuse and mental health assessments and counseling.
  6. Knowledge of evidenced based practices and therapeutic treatment models including Cognitive Behavioral Therapy, Motivational Interviewing, Seeking Safety, etc. 
  7. Experience in an outpatient clinic setting
  8. Ability to maintain patient confidentiality
  9. Strong commitment to the National Association of Social Workers (NASW) Code of Ethics including principles of social justice, empowerment and self-determination of individuals
  10. Ability to work well with others in team-oriented environment, work independently when needed, take initiative and use good judgment
  11. Bilingual English/Spanish
