The Community Health Worker/Care Management Support Professional 1 contributes to administration of care management. Provides non-clinical support to the assessment and evaluation of members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Management Support Professional 1 work assignments are often straightforward and of moderate complexity.Humana’s Community Health Workers (CHW) serve as members of our Louisiana Medicaid enrollee centric Comprehensive Care Support teams. The CHW applies both a hands-on approach, as well as telephonic outreach, to enrollee engagement to support case management functions, address social determinants of health (SDOH) needs, improve self-management of chronic conditions, navigate the healthcare system, and promote prevention and health education that is tailored to the needs of the communities we serve. The CHW leverages their knowledge of the community and shared life experience to inform their interactions with enrollees and Humana’s community partners. This role reports to theManager Care Management and be part of the Medicaid Team.

To ensure Home or Hybrid Home/Office employees’ ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria:
Workstyle: Combination remote work at home and onsite/home member visits
Hours: Must be able to work a 40 hour work week, Monday through Friday 8:00 AM to 5:00 PM, over-time may be requested to meet business needs and requires approval.
Screening: This role is considered patient facing and is part of Humana’s Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.Â
Travel, Driver's License, Transportation, Insurance: Must be willing to travel at least 75% of the time within a 30-40 mile radius of your assigned Region, to conduct field visits with members in your assigned area. Travel requirements may vary based on member tier level.
This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Â

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting.
