Location: Lexington Lowell, Chelmsford, Acton, and surrounding areas along the I-495 belt
Schedule: Full-Time | 40 hours per week
Compensation: $120,000 – $130,000 annually + performance-based bonus

Position Summary

We are currently seeking a Full-Time Physical Therapist (PT/RPT) to join our growing home care team. This role provides consistent full-time hours (40 hours per week), competitive compensation, comprehensive benefits, performance-based bonuses, and the opportunity to make a meaningful impact by delivering one-on-one care in patients’ homes.

The Physical Therapist will support patients across Lowell, Chelmsford, Framingham, Billerica, and surrounding communities along the I-495 belt, working closely with our skilled nursing and interdisciplinary care teams.

Key Responsibilities

  •  Provide comprehensive and compassionate physical therapy services in patients’ homes in accordance with physician orders 
  •  Perform initial evaluations and ongoing assessments of patients’ functional abilities 
  •  Develop and implement individualized treatment plans to improve mobility, strength, balance, and quality of life 
  •  Deliver skilled therapeutic interventions, exercises, and patient/caregiver education 
  •  Document patient progress accurately and in a timely manner per company and regulatory standards 
  •  Collaborate with nurses, caregivers, physicians, and interdisciplinary teams to ensure coordinated care 
  •  Maintain a safe, supportive, and professional therapy environment at all times 
Qualifications

  •  Graduate of an accredited Physical Therapy program 
  •  Current Physical Therapist (PT/RPT) license in Massachusetts 
  •  Prior experience in home care of 2 years, skilled nursing, or sub-acute rehab preferred but not required 
  •  Strong communication, interpersonal, and clinical documentation skills 
  •  Ability to work independently and manage a full-time caseload 
  •  Reliable transportation and ability to travel within assigned service areas
