Position Summary
We are currently seeking a Full-Time Physical Therapist (PT/RPT) to join our growing home care team. This role provides consistent full-time hours (40 hours per week), competitive compensation, comprehensive benefits, performance-based bonuses, and the opportunity to make a meaningful impact by delivering one-on-one care in patients’ homes.
The Physical Therapist will support patients across Lowell, Chelmsford, Framingham, Billerica, and surrounding communities along the I-495 belt, working closely with our skilled nursing and interdisciplinary care teams.
Key Responsibilities
-  Provide comprehensive and compassionate physical therapy services in patients’ homes in accordance with physician ordersÂ
-  Perform initial evaluations and ongoing assessments of patients’ functional abilitiesÂ
- Â Develop and implement individualized treatment plans to improve mobility, strength, balance, and quality of lifeÂ
- Â Deliver skilled therapeutic interventions, exercises, and patient/caregiver educationÂ
- Â Document patient progress accurately and in a timely manner per company and regulatory standardsÂ
- Â Collaborate with nurses, caregivers, physicians, and interdisciplinary teams to ensure coordinated careÂ
- Â Maintain a safe, supportive, and professional therapy environment at all timesÂ
Qualifications
- Â Graduate of an accredited Physical Therapy programÂ
- Â Current Physical Therapist (PT/RPT) license in MassachusettsÂ
- Â Prior experience in home care of 2 years, skilled nursing, or sub-acute rehab preferred but not requiredÂ
- Â Strong communication, interpersonal, and clinical documentation skillsÂ
- Â Ability to work independently and manage a full-time caseloadÂ
- Â Reliable transportation and ability to travel within assigned service areasÂ
