Job Title:  Admissions Coordinator

Department:  Carillon House Health Center                                                     

Reports to:  Health Center Administrator                                

FLSA Status:  Non-exempt

Hourly Rate: $22.00 - $25.00 

SUMMARY OF RESPONSIBILITIES

The Admissions Coordinator is responsible for managing the Health Center admissions process and for establishing and maintaining an open, effective communication between the facility and referral sources, physicians, and third party payers.

ESSENTIAL JOB FUNCTIONS

  • Obtains referrals from hospital social workers, physicians, or family members keeping wait lists current.
  • Functions as the Health Center public relations person, providing tours of the facility and pertinent information regarding admissions as well as occasional off-site visits to hospitals and doctor’s offices. 
  • Conducts admission interviews with the resident and/or responsible party, obtaining all required financial data, signatures, pertinent social information and medical information, attending physician information, and other pertinent and required documentation prior to admission.
  • Reviews all admission related paperwork with the resident and or/responsible party including Medicare Part A coverage and non-coverage procedures. Maintains all non-medical record related paperwork in an admissions file for each resident.
  • Processes all Medicare denial letters, Medicare certification and re-certifications according to Medicare regulations.
  • Maintains Life Care resident statistics, permanent assignments, temporary assignments, and notifies resident and/or the responsible party of permanent and temporary status in the Health Center.
  • Collects and reports data reflecting daily/monthly census of residents in the Health Center noting admissions, discharges, deaths, transfers, changes in payment methods, and temporary and permanent assignments.
  • Maintains an accurate list of number of days an independent living resident resides in the Health Center.
  • Coordinates utilization review for new admissions covered by Medicare Part A.
  • Maintains complete confidentiality regarding admission information and resident records.
  • Assists in coordinating admissions/transfers/discharges with other departments.
  • Ensures that the resident, family, and responsible person are fully informed of the resident’s personal and property rights.

QUALIFICATIONS:

Good working knowledge of Medicare Part A and certification procedures. Experience in a long-term care setting desired. Must possess excellent written, oral and interpersonal skills, as well as the ability to manage multiple priorities in a time-constrained environment.             

EDUCATION AND EXPERIENCE

  • Licensed Professional: Bachelor of Social Work / LVN
  • One year Admissions Coordinator experience in a health care setting preferred.
  • Requires good judgment to plan, perform and set-up sequences within standard procedures. Requires analysis of facts and information for action following established guidelines.

PHYSICAL REQUIREMENTS/WORKING CONDITIONS:

Normally works in a well- lighted, comfortable office setting. Must be able to work efficiently with frequent interruptions. Must be able to move freely throughout the facility and able to lift (unassisted) 50 lbs. May be subjected to offensive odors and exposure to chronically ill and frail elderly. Must be free from communicable disease.

EQUIPMENT USED

The Admissions Coordinator will use the following equipment: computer and various computer programs, copy machine, fax machine, calculator, and phone usage.

OSHA TASK CATEGORY III 

Tasks that involve no exposure to blood, body fluids, and/or tissues but Category I tasks are not a condition of employment.

COMPLIANCE 

Responsible for abiding by all Carillon policies and procedures to include the Code of Ethics and the principles and standards set forth for conducting the business and affairs of Carillon, Inc.

Health Insurance Portability and Accountability Act of 1996 (HIPAA)

The Admissions Coordinator will be responsible for abiding by all Carillon policies and procedures relating to HIPAA.
