This posting is for multiple openings of Care Management Coordinator - Case Management at Covenant Medical Center in Lubbock, TX. We are hiring for the following opportunities:

Full Time, Day Shift, 40 hours a week

***Please note that this position is onsite and therefore requires the ability to commute to Covenant Medical Center in Lubbock, TX.***

The Care Management Coordinator provides essential clerical support for the Care Management Department. Under the supervision of licensed clinical staff, accountabilities include coordinating communication between patients, health care providers, insurance companies and other stakeholders to ensure safe and efficient transitions of care, payment for clinical services provided and regulatory compliance. The goal of the Care Management Coordinator is to ensure the maintenance of accurate records, process documentation and support the delivery of quality patient care. This role requires excellent organizational skills, attention to detail, and the ability to handle multiple tasks simultaneously.

Required Qualifications:

2 years of administrative experience, preferably in a healthcare setting or demonstrated knowledge of healthcare terminology

Preferred Qualifications:

Bachelor's Degree or higher in Business, Healthcare, or related field

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
