BASIC PURPOSE OF THE JOB

The Facilitator organizes and facilitates classes provided by the Community Outreach Program.

 

ESSENTIAL FUNCTIONS

Prepared in advance on a weekly basis for each session, maintains a positive attitude and operates from a strengths perspective when working with other staff and participants

Ensure meeting sites are set up, lesson materials are available

Reports unusual family needs and concerns to the Program Manager/Director in a timely manner

Ensure that documentation and all requested paperwork is submitted on time

Serve as class facilitator for classes both in-person and virtually, including our jail program if applicable

Must be able to work evenings and Saturdays

Document all communication with clients in progress reports as well as nForm

Assist making, uploading, sending, certificates and welcome packets

Completion of entrance and exit surveys with clients

Complete makeup with clients if they miss class

Implements SFM policies and follows directives as required. Follows and adheres to all pertinent SFM Standard Operating Procedures (SOP’s), rules, personnel policies and procedures; related accreditation and licensure standards; and federal, state and local rules, statutes, regulations, and contractual terms

Is knowledgeable of and follows all safety procedures

Reports on unusual incidents through appropriate Risk Management, clinical and safety channels

Ensures clients’ rights are protected

Qualifications

MINIMUM REQUIREMENTS

High School Diploma or equivalent

First Aid/CPR

Must be 21 years of age

Must pass a drug screening

FBI Fingerprint Check

Submit to a Criminal History and DFPS History Check

Pass a Centralized Background Check through the Texas Health and Human Services Eligibility and any background checks deemed to be necessary

Must have valid driver’s license and reliable transportation

Lifting requirements of 50 lbs.

 

REQUIRED SKILLS, KNOWLEDGE, AND ABILITIES

Mandated reporter

Receives training in trauma informed practice and incorporates knowledge in day-to-day practice

Ability to work with minimal direct supervision

Ability to work a flexible schedule
