Schedule Details: Full Time, Evening Shift – 12-hour shifts every Sunday through Tuesday + 8 hour shifts every other Wednesday
Responsible for the coordination of care and mental health referral services for Covenant Health emergency departments, providing interventions for patients. Provides referral and connection to needed services for patients and families presenting to the emergency departments, working to prevent avoidable admissions/readmissions. Provides assessment and appropriate interventions for patient care needs. Works to ensure appropriate utilization of acute care beds through assessment of alternative site of care options. Collaborates with medical care teams, community agencies and patients/families to ensure safe discharge planning. Works to determine alternative site of care options and initiates proactive discharge planning for patients being admitted. Assists multidisciplinary team to recognize and respond to mental health needs through assessment and disposition. Responsible for entering into the database/tracking and all EMR documentation of services provided.

Required Qualifications:
- Bachelor's Degree in Social Work, Psychology, or related healthcare field
- National Provider BLS - American Heart Association upon hire
- 3 years of experience working with children and families, preferably in child protection system, mental health clinic or agency
