Job Summary/Basic Functions:

The Community Outreach Coordinator is responsible for developing and nurturing relationships with community partners, increasing awareness of the organization's substance use counseling services, and driving referrals through strategic outreach initiatives. This role bridges the gap between the community and individuals in need of support, playing a vital role in connecting people to care and promoting recovery resources.



Job Description/Responsibilities:

  • Reports to the Clinical Director for clinical supervision required to maintain licensure.
  • Directly responsible for maintaining bed occupancy.
  • Responsible for informing referral sources of bed vacancy and diligently seek to fill vacant beds.
  • Enhances organization reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.
  • Counseling and Case Management duties as assigned.
  • Maintains professional conduct and dress during outreach, court, probation and community events.
  • Provides crisis management for clients as needed.
  • Assists with linkages for interventions as appropriate.
  • Ability to demonstrate competent use of computer programs needed to carry out job task; use of PC computers, Apple computers, cell phones, etc in order to maintain communication with referral resources.
  • Attend all employee staffing when available and meet regularly with the Admissions Manager to suggest changes to admission criteria, service improvement areas, etc.
  • Attend meetings as appropriate and meet regularly with Clinical Director for required supervision.
  • Obtains applicant information by assisting with and/or requesting completed applications and medical information; verifying and clarifying information; obtaining insurance information, interviewing clients and family members; explaining admission criteria and processes.
  • Screens clients by comparing client's condition to admission criteria; evaluating and accepting or rejecting clients; referring clients and family to other programs and institution with proper documentation.
  • Report all potential ineligible clients to the clinical director and human resources -immediately upon discovery of ineligibility for further investigation for alternate program placement.
  • Coordinating and arranging physical, social, emotional, and support service requirements, including transportation to and from Focus facilities to needed services.
  • Recommends and suggests changes in service policies and procedures to better improve services.
  • Keeps clients safe by following safety policies, procedures, and regulations.
  • Develop and implement outreach strategies to promote substance use counseling services across targeted communities.
  • Cultivate partnerships with local healthcare providers, schools, social service agencies, faith-based organizations, and law enforcement.
  • Represent the organization at community events, health fairs, coalition meetings, and networking functions.
  • Coordinate and lead presentations, workshops, and informational sessions to raise awareness about substance use disorders, treatment options, and prevention.
  • Serve as a liaison between referring agencies and the counseling team to ensure seamless client transitions.
  • Track and report outreach activities, referral trends, and community engagement outcomes.
  • Collaborate with human resources and executive director to develop outreach materials and social media content aligned with public education and engagement goals.
  • Stay informed about local substance use trends, community needs, and relevant public health initiatives.
  • Assist in the planning and promotion of recovery-related events such as National Recovery Month, Overdose Awareness Day, and local town halls.
