As an Admissions RN, you'll serve as the clinical leader who ensures every patient begins home health with confidence, clarity, and the right plan for success.

What You'll Do
- Complete comprehensive Start of Care (SOC) admissions throughout the Twin Cities Metro.
- Perform detailed nursing assessments and complete accurate OASIS documentation.
- Develop individualized plans of care that set patients up for long-term success.
- Educate patients and families on diagnoses, medications, services, and expectations.
- Coordinate seamlessly with physicians, case managers, therapists, and the interdisciplinary care team.
- Ensure smooth, safe transitions from hospitals, skilled nursing facilities, and clinics into home health.
What We're Looking For
- Active Minnesota Registered Nurse (RN) license.
- Strong clinical assessment and critical thinking skills.
- Home health and OASIS experience strongly preferred.
- Exceptional communication and patient education abilities.
- Ability to work independently and manage a field-based schedule.
- Reliable transportation.
