GENERAL SUMMARY: 

The LICSW Supervisor  provides short-term, intensive clinical intervention, care planning, & leadership. Managing a dynamic, rotating caseload for members requiring elevated psychosocial support & targeted intervention for SDO).

Receiving internal referrals directly from RN Case Managers, the LICSW provides rapid short-term stabilization through home visits, comprehensive social service care planning, advanced benefits management, & community resource linkage. This position also provides direct administrative & clinical supervision to Community Health Workers (CHW).

Essential Responsibilities:

  • Develops, implements, and monitors comprehensive, short-term care plans specifically designed to address acute behavioral health & complex social service needs.
  • Evaluates & accepts internal clinical referrals from RN Case Managers for members experiencing severe environmental, financial, or psychosocial barriers.
  • LICSW Supervisor conducts regular home visits across the designated service area to assess safety, environment, & social needs. 
  • Facilitates a safe transition away from social work services, returning care to the primary RN Case Manager as localized social & emotional stability is achieved.
  • Directly targets critical social needs including housing instability, food insecurity, utility shut-offs, & environmental safety. 
  • Systemic Member Advocacy: Advocates for member rights, health equity, & cultural preferences within the Interdisciplinary Care Team (ICT) & regional healthcare systems.
  • Benefits Management: Guides members through complex state & federal benefit landscapes, actively assisting them to secure, maintain, or maximize MassHealth, Medicare, SNAP, fuel assistance, & other critical entitlement programs. 
  • Community Service Access: Maps & links members to vital community resources, including Elder Protective Services, Aging Services Access Points (ASAPs), & community mental health clinics. 
  • CHW Supervision: Supervises, mentors, and evaluates Community Health Workers (CHWs) dedicated to field-based & telephonic member support.
  • Task Delegation: Directs CHW staff in executing tasks related to SDOH support, gathering documents, application submission, & resource follow-up.
  • Recruits, develops, mentors and motivates staff. Initiates and communicates a variety of personnel actions including employment, termination, performance reviews, salary reviews, and disciplinary actions.

Job Specification:

  • Licensed Independent Clinical Social Worker required from an accredited institution.
  • Current, active, & unrestricted Licensed Independent Clinical Social Worker (LICSW) in the Commonwealth of Massachusetts.
  • Minimum of 5 years of clinical social work experience dedicated to working with the 65+ older adult/geriatric population.
  • Bilingual fluency in English & Spanish is highly preferred to effectively serve our diverse member population.
