Director of Community Living is responsible for providing leadership, direction, and clinical and administrative oversight for Bridgeway’s Community Living programs in Pekin and Macomb, including DD CILA Housing Services and HUD Housing. Ensures effective implementation of Quality Assurance processes and supports the overall Quality Management system for both sites. Monitors compliance with Bridgeway policies and CARF, Medicaid Waiver, and DHS Rules 115, 119, and 116. Maintains clear communication with staff, strengthens community relationships, and promotes awareness of services for individuals with disabilities while identifying opportunities for program growth. The Director leads with a person centered approach, fostering meaningful community connections and guiding staff to provide high quality support. This role ensures individuals served gain greater independence in daily living, community integration, socialization, health, wellness, and safety.  To learn more about this position, please read the summarized list of duties below: Coordinates, monitors and ensures that agency corporate compliance and performance standards related to service provision and clinical documentation are adhered to, including but not limited to the following: Entry, Transition and Exit of consumers to/from services. Assessments, Consumer Centered Plans/Implementation Strategies and other documentation is monitored to ensure provision of sufficient rationale to support the diagnosis/medical necessity and the type, frequency and duration of services provided.   Reviews of documentation and consumer records on a regular basis. Person Centered Individualized Service Planning Process  Discharge Planning HUD and other housing requirements, as applicable Oversee staff productivity in coordination with Coordinators, assisting with time management and coaching staff to achieve productivity/performance expectations. Ensure clinical oversight of the consumer cases either providing directly or facilitating clinical consultation through the interdisciplinary team process. Work closely with the Director of Community Support Services to ensure consumers services are in line and cross train in that position as well.  Coordinate Quality Assurance activities for the Macomb and Pekin based DD services regularly monitoring consumer records to ensure compliance with applicable standards and serve on the Quality Assurance Committee. Maintain program effectiveness, efficiency, and accessibility data, as well as consumer and stakeholder satisfaction data contributing to annual Quality Improvement reports, with a focus on continuous quality improvement. Collaborate with other management staff to establish high quality service systems which ensure provision of comprehensive assessments, Implementation Strategies and Person Centered Plans/ Behavior Support Plans designed to meet consumer’s individual needs. Assume primary leadership and coordination of the Representative Payee services for both Macomb and Pekin overseeing the staff responsible for the day to day management of the Representative Payee Services. Minimum requirements/qualifications: Master’s or Bachelor’s Degree in human service, or a registered nurse/nurse trainer or a DSP with a minimum of 10 years of service to people with intellectual/developmental disabilities; minimum of three years of administrative, supervisory and clinical experience in the field of disabilities; working knowledge of State regulations and the process of accreditation; Must have DSP (Direct Service Professional) status and Mental Health Professional (MHP) status. Must have reliable transportation to travel. Must have a valid driver's license, proof of insurance, and meet company insurability standards by having a good driving record. Must be able to travel to assigned Bridgeway's locations, community and Bridgeway events.  Employment with Bridgeway is conditional upon completion of all background checks, maintain required certifications and regulatory requirements.
