Full-Time Administrative LPN

Office hours Monday - Friday 8 am to 4 pm. Occasional evening and weekend work required.

DUTIES:

Create plan of care for all new individuals served
Conduct supervisory visits and document results
Annual oversight of health, risk and safety assessments using the Health Risk Screening Tool (HRST)
Review of data tracking and documentation
Provide staff training and oversight
Coordinate medical visits for clients
Medication passes
Other duties as assigned
