Position Overview:

River Edge is actively searching for a Full-time Case Manager to join our Intellectual and Developmental Disabilities department. In this role, you will be 
Coordinate and attend ISP and monthly home meetings, train direct support staff, assist with financial management, and provide client counseling and transportation. Develop staff schedules, approve timesheets, advocate for residents, and ensure compliance during audits, with on-call duties one week per month.

Duties & Responsibilities

  • Attend ISP meetings annually at the day center.
  • Hold monthly home meetings.
  • Train direct support staff on day one of in-home orientation.
  • Assist Lead with financial management of individual funds.
  • Observe and monitor client behavior to provide feedback in ISP meetings.
  • Counsel and advise clients on behavioral problems, daily decision-making, and the resolution of minor problems as needed.
  • Provide transportation for individuals to medical appointments as needed.
  • Act as advocate for residents with Support Coordinators, other providers, and the community as needed.
  • Maintain a flexible schedule to cover staff callouts and holiday coverage as needed.
  • Approve timesheets weekly, ensuring time is flexible to not work over 40 hours.
  • Develop the monthly staff schedule for the home.
  • Meet with lead staff weekly for coaching and mentoring.
  • Be present during audits from external organizations (HFR, Delmarva, GMCF, etc.).
  • Ensure CAPs are followed up in each home.
  • Communicate with family members as needed.
  • Be on call once per month.

Qualifications

  • Bachelor’s degree in social service field (Social Work, Sociology, Psychology, Human Services) or related field from an accredited college or university.
  • At least one (1) year experience working with behavioral health clients (mental health, substance abuse, developmental disabilities).
  • Must have a valid GA Driver’s License.
  • All applicants must pass a satisfactory background clearance and pre-employment drug test.
  • Provide 2 Professional references (Former employers only, no family members)
