What You’ll Do

As a Home Health RN Case Manager, you will independently manage a patient caseload, ensuring they receive timely, high‑quality care in accordance with individualized care plans and physician orders.

You will:

  • Conduct comprehensive patient assessments in the home setting and via telephonic outreach.
  • Analyze patient data to identify needs, risks, and resources for optimal outcomes.
  • Identify high‑risk patients (falls, rehospitalization, high utilization) and coordinate necessary interventions.
  • Prioritize multiple clinical issues based on urgency, safety, and patient outcomes.
  • Identify barriers such as language or literacy and collaborate with resources to resolve them.
  • Develop, implement, and oversee a patient‑centered Plan of Care.
  • Coordinate and direct the multidisciplinary home care team, including LPNs and HHAs.
  • Perform treatments and procedures per state regulations and agency standards.
  • Evaluate progress toward goals and adjust the care plan as needed.

Physical Requirements

This role requires the ability to walk, stand, lift up to 35 lbs, bend, reach, push, and pull. Must be able to respond quickly to changes in patient condition and communicate effectively in English. Physical Abilities Testing required.

Qualifications

  • Graduate of an accredited School of Nursing (BSN preferred)
  • Current RN licensure (state-specific or compact, as applicable)
  • Current BLS certification
  • Additional department‑specific training or certifications may be required
