What You’ll Do

  • You will evaluate each patient’s physical abilities and functional limitations through comprehensive assessments and diagnostic tests.
  • You will develop personalized plans of care that target functional improvement, safety, and independence—modifying as needed in consultation with the physician.
  • You will deliver hands-on therapy in the home, using evidence-based techniques to help patients meet their mobility and strength goals.
  • You will educate patients and caregivers on therapeutic exercises, safe mobility, and the use of prosthetics, orthotics, and adaptive equipment.
  • You will document progress accurately and promptly in the patient’s clinical record, maintaining clear communication with physicians and the interdisciplinary team.
  • You will ensure a safe environment for patients by practicing proper equipment handling, adhering to safety protocols, and collaborating with other care providers.

 

What You Bring

  • You are a graduate of an accredited Physical Therapy program; DPT preferred.
  • You hold a current, valid Physical Therapy license issued by the New York State Department of Education.
  • You have at least one year of post-licensing, practical experience as a licensed physical therapist.
  • You are a clear communicator, with excellent critical thinking, and problem-solving skills.
  • You have a passion for helping others regain confidence and independence through compassionate care.
