 Responsibilities/Accountabilities 

We have an amazing opportunity for Social Worker that can: 

  • Acts as facility liaison with county social workers, community services boards, Medicaid, SSI, Social Security, legal aide and other services
  • Performs assessments of residents 

 

Experience/Skills/Education 

Required: 

  • Master''s degree (M.S.W.) in Social Work  from accredited University and a minimum of five years related experience and/or training, or equivalent combination of education and experience.
  • Birmingham Green can offer free supervision to become LCSW, or LPC to the qualify Candidate

  • Experience working with the elderly in an adult living or long term care facility with knowledge of geriatric mental health services 

  • Knowledge of Medicaid/Medicare eligibility requirements, PDPM, MDS, Care Planning and basic medical terminology is required.
  •  Proficiency in computer skills, including MS Office, and Outlook
