Job Description:

We are seeking a Community Care Coordinator for our senior services program.

Schedule: Full time 40 hours a week Monday-Friday

Compensation: $62,500 annually

Job Responsibilities:

  • Assess needs of at-risk individuals who have spent excessive time in Urgent Care and/or the use of other emergency services, including unmet needs, service gaps and barriers. Serve as a “one stop” resource for coordination of care.  Assist in accessing necessary services. 

  • Work in collaboration with other service providers to maximize use of existing resources.

  • Coordinate care delivery mechanisms with client, caregivers, providers and others.

  • Complete assessments that may include, but are not limited to, outreach progress notes, intake assessments, case progress notes, care plans, data collection, and provide positive outcome summaries.

  • Participate in all relevant meetings, conferences, and sub-committees as assigned.

  • Educate service providers and others about specific issues that may be impacting client's needs.

  • Establish rapport with other care providers and facilitate meetings as needed to identify and resolve unmet needs, service gaps and barriers. Strategize possible systems change solutions to services.

  • Access ancillary service providers and preexisting programs whenever possible; provide services directly when no other resources are available or when direct service from Community Care Coordinator is indicated.

  • Maintain a positive working relationship with both inter- and intra-Agency sources.

  • Maintain professional ethics and adhere to the NASW code of ethics.

  • Retain state certification and/or licensure as applicable and  continue to develop professional skills by attending in-services and conferences.

Qualifications:

  • Master’s degree required, in Social Work or related field

  • Five (5) years’ experience providing social work services to the respective community

  • Demonstrate a genuine interest and understanding in working with the at-risk populations

  • Possess ability to function as an integral member of a multi-disciplinary administrative team

  • Valid driver’s license, auto insurance, and reliable transportation
