About the Role:

The RN Case Manager - Home Hospice plays a critical role in delivering compassionate, patient-centered care to individuals facing life-limiting illnesses in their homes. This position involves comprehensive assessment, care planning, and coordination of hospice services to ensure patients' comfort and dignity during end-of-life care. The RN Case Manager collaborates closely with interdisciplinary teams, patients, and families to address physical, emotional, and psychosocial needs. They serve as a primary clinical resource, advocating for patient preferences while managing symptom control and quality of life. Ultimately, this role ensures seamless hospice care delivery that aligns with regulatory standards and ethical practices across diverse home settings.

Minimum Qualifications:

  • Current and valid Registered Nurse (RN) license in the United States.
  • Minimum of two years of clinical nursing experience, preferably in hospice, palliative care, or related fields. New graduates may be considered.
  • Ability to work independently in a home care setting with excellent organizational and time management skills.
  • Proficiency in electronic medical records (EMR) and documentation practices.

Responsibilities:

  • Conduct thorough patient assessments to evaluate physical, emotional, and psychosocial needs in the home environment.
  • Develop, implement, and regularly update individualized care plans in collaboration with interdisciplinary hospice teams.
  • Coordinate and communicate effectively with physicians, social workers, chaplains, and other healthcare professionals to optimize patient care.
  • Provide education and support to patients and their families regarding disease progression, symptom management, and available resources.
  • Monitor patient status closely, adjusting care plans as necessary to manage symptoms and maintain comfort.
  • Ensure compliance with hospice regulations, documentation standards, and quality assurance protocols.
  • Facilitate timely admissions, transfers, and discharges while maintaining continuity of care.
  • Serve as a patient advocate, respecting cultural, spiritual, and personal preferences throughout the hospice experience.
