Job Description Summary:

This position is intended to provide coverage on-site to Mansfield ED and/or Marion ED, as needs arise. This position will not have set shifts, but rather be available to fill-in for full-time staff during time off or other open shifts in the schedule.

Previous work with crisis and/or inpatient behavioral health is required.

Responsibilities And Duties:

30%
Conducts patient/family/significant other interviews, charts reviews and professional collaboration to obtain and interpret information pertaining to patient biopsychosocial functioning. Completes assessments including: risk factors of harm toward self or others, developmental and age-specific needs, ability to care for self, ability to care for dependents, ability to meet basic custodial and financial needs, ability to engage in meaningful social and emotional relationships, and ability to comprehend and comply with treatment recommendations. Develops preliminary diagnosis formulation.
30%
Develops and implements plan of intervention based on assessment. Provides crisis intervention, supportive, educational, individual, couples, family or group therapies in an effort to resolve problem areas. Employs treatment planning, continuum of care planning, and discharge planning.

Maintains awareness of current hospital and community services to assist patient psychosocial functioning. Provides information and facilitates successful linkages between these services and patient.

Thoroughly documents assessments and interventions in patient's medical record and information systems in accordance with department standards. Complies with internal and external documentation requirements as related to specific patient/system needs and regulatory requirements.

Minimum Qualifications:

Master's Degree (Required)DL-HC - Driver's License only if in a Home Care department - Department of Motor Vehicles, LSW - Licensed Social Worker - Social Work Certification and Licensure Board

Additional Job Description:

Social Work, Counseling or related field; Licensure to practice independently in the State of Ohio, e. g. , LISW, LPCC. Clinical psychopathology, psychiatric diagnosis and treatment. Demonstrates knowledge of bio-psychosocial development for the age of patients served. KIND and LENGHTH of Experience working with acutely-ill psychiatric patients.
